The 2009 Statistical and Information Forms, to be completed by pastoral charges, were mailed to charges were mailed to charges January 8-14, 2010. If you have not received your forms by the end of the month, please contact the Information and Statistics Coordinator at the number below.

This year’s forms are accompanied by a Congregational Resources Survey for each congregation on the pastoral charge. The Congregational Resources Survey can be completed online—you will need the congregation ID number(s) that are included in the mailing.

We are also piloting an online reporting option for the 2009 Statistical Forms, which we hope to have ready by February 15, 2010. This option would allow charges to log on to a website and submit their information electronically. To do this, you will need to have a Microsoft Internet Explorer browser.

Please complete and return the five blue forms—sections 1, 2-3, 4, 5, and 6—as well as the resources surveys to the General Council Office in the return envelope provided (or online), along with congregational or pastoral charge Annual Reports for 2008, if available. The deadline for returning the forms is

• Monday, March 1, 2010 (or earlier) for single-point pastoral charges
• Friday, March 12, 2010 (or earlier) for multi-point pastoral charges
